RN Case Manager

Work Arrangement: Full Office
Schedule: Monday–Friday, 8:00 AM–5:00 PM
Assignment: Up to 60 days, with possible extension based on business needs, not to exceed 6 months
Pay Range: $43.66–$56.77/hour

Job Summary

We are seeking a Registered Nurse – Case Management to provide care management and support to members with complex medical, behavioral, and social needs.

The RN will conduct assessments, coordinate care, develop individualized care plans, work with healthcare providers and families, and advocate for appropriate services and care.

Responsibilities

• Conduct in-field, virtual, and telephone assessments.
• Assess members' physical, psychosocial, and environmental needs.
• Develop and update individualized care plans.
• Coordinate care with physicians, healthcare professionals, families, and community agencies.
• Monitor members' progress and adjust care plans as needed.
• Respond to member crisis situations.
• Review medical and clinical documentation.
• Support quality-of-care and quality-of-life improvements.
• Advocate for appropriate benefits, treatment options, and access to care.

Minimum Qualifications

• Associate Degree in Nursing (ADN).
• At least 3 years of clinical experience working with the health needs of the population served.
• Current unrestricted California RN license.
• Valid driver's license and reliable vehicle, with acceptable driving record and current auto insurance, or another approved means of transportation, as the role may require travel away from the primary office 50% or more of the time.

Preferred Qualifications

• BSN.
• Case Management Certification (CCM).
• Public Health Nurse (PHN) certification.
• Experience in home health, hospice, or skilled nursing.
• Experience with Level of Care Certification processes.
• Bilingual in English and Arabic, Farsi, Chinese, Korean, Russian, Spanish, or Vietnamese.
